We are seeking a passionate and dedicated Social Service Admin to support residents who face high-acuity situations, often dealing with mental health issues, substance abuse, and rehabilitation. This role is crucial in providing support and resources to help them navigate these challenges.

Responsibilities:

• Coordinate community engagement events and outings ensuring active and meaningful participation.

• Provide case management services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkage to community providers for substance use, primary and mental health care.

• Collaborate with clients to develop individualized treatment plans that address both short- and long-term goals.

• Offer ongoing support to assess plan progress and adjust goals as necessary.

• Maintain documentation standards as set forth by the program contract.

Qualifications:

• Strong commitment to helping individuals facing high-acuity situations.

• A minimum of 1 year experience in case management or similar experience.

• Ability to work collaboratively with clients to develop and achieve treatment goals.

• Experience with HMIS is a plus

• Must be comfortable working in an interim housing shelter, and harm reduction.

Available to work onsite Monday to Friday 8am-5pm
